এজি অফিসে আয়কর হিসাবে কর্তনকৃত অর্থের বাৎসরিক হিসাব প্রতিপাদন করে আনতে নিম্নের ফরমটি প্রয়োজন পড়বে।

এরপর এজি অফিসে জমা দিয়ে একাউন্ট অফিসার দিয়ে স্বাক্ষর করে নিয়ে আয়কর অফিস জমা দেয়া যাবে।
