সরকারি চাকরিজীবী পেনশন ভোগরত অবস্থায় মৃত্যুবরণ করলে আমরা অনেকেই বুঝতে পারিনা কিভাবে কি করতে হবে। কি কি কাগজপত্র লাগবে, ইত্যাদি ইত্যাদি। নিচের তালিকা মোতাবেক কাগজপত্র সংযুক্ত করে একটি আবেদন পত্র যথাযথ অফিস কর্তৃপক্ষের মাধ্যমে হিসাব রক্ষণ অফিসে দাখিল করলেই কাজ শেষ। কিছু কাগজপত্র অফিস কর্তৃপক্ষ রেডি করে দিবে আর কিছু কাগজপত্র পরিবারকে যোগার করতে হবে।

(১) পেনশন আবেদন ফরম ২.২ (সংযোজনী-৫)- ১ কপি।

(২) সত্যায়িত ছবি- ৪ কপি।

(৩) উত্তরাধিকার সনদপত্র ও নন-ম্যারিজ সার্টিফিকেট (সংযোজনী-৩)- ৩ কপি।

(৪) নমুনা স্বাক্ষর ও হাতের পাঁচ আঙ্গুলের ছাপ (সংযোজনী-৬)- ৩কপি।

(৫) অভিভাবক মনোনয়ন এবং অবসর ভাতা ও আনুতোষিক উত্তোলন করিবার ক্ষমতা অর্পণ সনদ (সংযোজনী-৭)- ৩ কপি।

(৬) চিকিৎসক/পৌরসভা/ইউনিয়ন পরিষদ চেয়ারম্যান/কাউন্সিলর কর্তৃক প্রদত্ত মৃত্যু সনদপত্র- ১ কপি।

(৭) পিপিও এবং ডি-হাফ-১কপি।

 

উক্ত কাগজপত্র ব্যতীত হিসাব রক্ষণ অফিস অতিরিক্ত কোন কাগজপত্র চাহিতে পারিবে না।
